Enrollment

Pathways uses a holistic, team process to determine if the Wellness Center is best for each student:

  1. Referral: A form to be completed by a student, school or a family member. 2024-2025 PWC Referral Form
  2. Family Meeting: A personalized conversation with staff at the Wellness Center to understand the family's needs and how the center can best meet those needs
  3. Wellness Center Review Team: The referral application will be reviewed by the director, clinical specialist, nurse and counselor to determine if the center is the right fit for the student
  4. Enrollment: Pathways will notify the family of enrollment (or other options) and work with their high school and family on transitioning to the center.  We try and make this a quick process.
